Terms and Conditions - Teachers Deserve Healthy Smiles

PLEASE READ THESE TERMS CAREFULLY BEFORE SUBMITTING A NOMINATION.

Participation in this Contest constitutes acceptance of all terms and conditions stated herein. This Contest is subject to all applicable federal, state, and local laws. Void where prohibited.

1. SPONSOR

This contest (the "Contest") is sponsored by Family and Implant Dentistry of Stuart ("Sponsor"), located at 2385 SE Federal Hwy, Stuart, FL 34994. The Contest is not sponsored, endorsed, administered by, or associated with any government agency, employer, or school department.

2. CONTEST NAME AND DESCRIPTION

The Contest is titled "Teachers Deserve Healthy Smiles." The purpose of this Contest is to recognize and honor active teachers in the Stuart and Martin County, Florida area by awarding one (1) Grand Prize winner with dental treatment services provided by Sponsor, valued at up to Ten Thousand Dollars ($10,000.00).

3. CONTEST PERIOD

Nomination Period Opens: May 15, 2026 at 12:00 a.m. Eastern Time (ET)
Nomination Period Closes: June 15, 2026 at 11:59 p.m. Eastern Time (ET)
Winner Selection Date: June 25, 2026
Winner Announcement Date: June 26, 2026

Sponsor reserves the right to extend or modify the Contest Period at its sole discretion. Any such changes will be posted at Family & Implant Dentistry of Stuart .

4. ELIGIBILITY

To be eligible to be nominated and to win, a nominee must meet ALL of the following criteria at the time of nomination and at the time of prize award:

  • Be a current, active educator — including classroom teachers (K–12) employed at a public or private school within Martin County, Florida;
  • Be actively employed in that role at the time of nomination and at the time of prize award (substitute teachers and contracted third-party staff are not eligible);
  • Be at least eighteen (18) years of age or older at the time of nomination;
  • Be a legal resident of the State of Florida;
  • Not be an employee, officer, director, agent, or immediate family member (defined as spouse, parent, child, or sibling, whether by blood, marriage, or adoption) of the Sponsor, its affiliates, subsidiaries, advertising agencies, or prize vendors;
  • Not have previously won a prize from Sponsor valued at $500 or more within the preceding twelve (12) months;
  • Be willing and medically able to receive the awarded dental treatment as determined by a licensed dentist employed by or contracted with Sponsor.

Individuals may not nominate themselves. A nominator must be a current colleague or supervisor within the same department or agency as the nominee. Each department may submit no more than one (1) nomination.

5. HOW TO ENTER

No purchase is necessary to enter or win. A purchase does not improve the chances of winning.

To submit a nomination, a nominator must complete the official nomination form available at Teachers Deserve Healthy Smiles during the Contest Period. The following information is required:

  • Nominator's full legal name, email address, relationship to nominee (colleague, administrator, or parent/guardian) school name;
  • Nominee's full legal name, school name, grade level or subject taught, and years of service in education;
  • Nominee's email address and phone number (for winner notification);
  • A written statement of up to two hundred fifty (250) words explaining why the nominee deserves recognition.

Nominations submitted by any means other than the official online form will not be accepted. Nominations that are incomplete, illegible, contain false or fraudulent information, or are received outside the Contest Period will be disqualified. Sponsor is not responsible for lost, late, misdirected, or incomplete nominations.

By submitting a nomination, the nominator represents and warrants that all information provided is accurate and that they have obtained the nominee's consent to submit their personal information.

6. PRIZE

One (1) Grand Prize will be awarded. The Grand Prize consists of dental treatment services provided exclusively by Family and Implant Dentistry of Stuart with a total value of up to Ten Thousand Dollars ($10,000.00) ("Prize").

The specific dental treatments included in the Prize will be determined by a licensed dentist employed by or contracted with Sponsor following a complimentary dental examination of the winner. Eligible treatments may include, but are not limited to, any combination of the following:

  • Invisalign orthodontic treatment (full or Lite)
  • Dental crowns
  • Porcelain veneers
  • Teeth whitening
  • Composite bonding
  • Other restorative or cosmetic dental services as determined appropriate by Sponsor's licensed dentist

The Prize is subject to the following conditions and limitations:

  • The Prize has no cash value and may not be redeemed for cash, transferred, assigned, or substituted, except at Sponsor's sole discretion.
  • The Prize must be accepted as awarded. No substitutions will be made except by Sponsor due to unavailability of a specific treatment, in which case Sponsor reserves the right to substitute a treatment of equal or greater value.
  • Treatment must be received at Family and Implant Dentistry of Stuart, Stuart, Florida. No other location will be honored.
  • Treatment must be scheduled and initiated within one hundred eighty (180) days of the winner's acceptance of the Prize. Failure to initiate treatment within this period may result in forfeiture of the Prize.
  • The Prize covers the cost of treatment up to $10,000.00 as billed at Sponsor's standard rates. Any treatment recommended or requested beyond the $10,000.00 cap is the sole financial responsibility of the winner.
  • The Prize does not include transportation, lodging, time off from work, or any other incidental expenses.
  • Winner is solely responsible for all applicable federal, state, and local income taxes on the Prize. Sponsor will issue an IRS Form 1099-MISC to the winner if the fair market value of the Prize exceeds $600.00 in the calendar year awarded.
  • Winner must be deemed a suitable candidate for the awarded treatment(s) by Sponsor's licensed dentist. If the winner is not a suitable candidate for any reason, Sponsor may substitute an alternate treatment of comparable value or, at Sponsor's discretion, select an alternate winner.

7. WINNER SELECTION

The winner will be selected by a panel of Sponsor's staff (the "Selection Panel") from all eligible nominations received during the Contest Period. The Selection Panel will evaluate nominations based on the following criteria:

  • Strength, sincerity, and specificity of the nomination statement;
  • Years of service and demonstrated commitment to students and the Martin County community;
  • Overall dental need as described in the nomination or determined at examination;
  • Representation across different schools and grade levels (at Sponsor's discretion, to ensure broad recognition).
  • The decisions of the Selection Panel are final and binding in all matters related to this Contest. The Contest is a contest of merit and is not a random drawing or sweepstakes.

8. WINNER NOTIFICATION AND VERIFICATION

The selected winner will be contacted privately via the email address or phone number provided in the nomination form prior to any public announcement. The winner will be required to:

  • Respond to the notification within five (5) business days of issuance. Failure to respond within this period may result in disqualification and selection of an alternate winner;
  • Provide proof of active employment as a teacher in the eligible service area (e.g., valid school-issued ID, letter from school, or current pay stub);
  • Provide valid government-issued photo identification confirming name, age, and Florida residency;
  • Execute and return an Affidavit of Eligibility, Liability Release, and (where permitted by law) a Publicity Release within ten (10) business days of receipt;
  • Attend a complimentary dental examination at Sponsor's Stuart office prior to initiation of any treatment.

Sponsor reserves the right to verify the eligibility of any nominee or winner. Any winner who cannot be verified, does not respond within the required timeframe, is found to be ineligible, or declines the Prize will be disqualified. Sponsor may select an alternate winner from remaining eligible nominations at its sole discretion.

9. PUBLICITY

Except where prohibited by law, acceptance of the Prize constitutes the winner's consent to Sponsor's use of the winner's name, photograph, likeness, voice, biographical information, and statements regarding the Prize for advertising, promotional, and marketing purposes in any and all media now known or hereafter devised, worldwide, in perpetuity, without further compensation, notice, or approval.

Sponsor will not publicly announce the winner's name without the winner's prior written consent. A separate Publicity Release will be provided to the winner at the time of notification.

10. CONDITIONS OF PARTICIPATION

By participating in this Contest (whether as a nominator or nominee), all participants agree to:

  • Be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all respects;
  • Release and hold harmless Sponsor, its affiliates, subsidiaries, officers, employees, agents, and representatives from any and all claims, liability, damages, or causes of action arising out of participation in this Contest or the acceptance, use, or misuse of any Prize awarded;
  • Grant Sponsor permission to collect and use personal information submitted in the nomination form for purposes of administering this Contest, as further described in Sponsor's Privacy Policy available at Privacy Policy .

Sponsor reserves the right to disqualify any participant or nominee who tampers with the nomination process, violates these Official Rules, or acts in a manner that is disruptive, deceptive, fraudulent, or detrimental to the Contest or to other participants. Sponsor reserves the right to cancel, suspend, or modify the Contest if fraud, technical failures, or any other factors beyond Sponsor's reasonable control impair the integrity of the Contest.

11. LIMITATION OF LIABILITY

SPONSOR MAKES NO WARRANTIES, EXPRESS OR IMPLIED, WITH RESPECT TO THE PRIZE, INCLUDING BUT NOT LIMITED TO ANY IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. SPONSOR IS NOT RESPONSIBLE FOR ANY INJURY, LOSS, OR DAMAGE TO PERSONS OR PROPERTY ARISING FROM PARTICIPATION IN THIS CONTEST OR FROM THE RECEIPT, USE, OR MISUSE OF ANY PRIZE. SPONSOR'S TOTAL LIABILITY TO ANY PARTICIPANT OR WINNER SHALL NOT EXCEED THE FAIR MARKET VALUE OF THE PRIZE AWARDED TO THAT INDIVIDUAL.

In the event of a dispute regarding the identity of a nominator or nominee, the authorized account holder of the email address used to submit the nomination will be deemed the nominator. "Authorized account holder" means the natural person assigned to an email address by the internet service provider, online service, or organization responsible for assigning email addresses.

12. GOVERNING LAW AND DISPUTES

This Contest is governed by the laws of the State of Florida, without regard to conflict of law principles. Any dispute, claim, or controversy arising out of or relating to this Contest or these Official Rules shall be resolved exclusively by binding arbitration administered by JAMS under its Streamlined Arbitration Rules, with proceedings to be conducted in Martin County, Florida. The arbitrator's decision will be final and binding. Each party will bear its own costs and attorneys' fees unless the arbitrator finds that a claim was frivolous or brought in bad faith.

Notwithstanding the foregoing, Sponsor reserves the right to seek injunctive or other equitable relief in any court of competent jurisdiction to prevent the actual or threatened misappropriation of its intellectual property or confidential information.

13. NO PURCHASE NECESSARY STATEMENT

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS CONTEST. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. THIS CONTEST IS VOID WHERE PROHIBITED OR RESTRICTED BY LAW.

14. PRIVACY

Personal information collected through this Contest will be used solely for the purpose of administering the Contest, notifying and verifying the winner, and (with the winner's separate consent) for promotional purposes. Sponsor will not sell or share personal information with unaffiliated third parties except as required by law or as necessary to administer the Contest (e.g., to a prize vendor). 15. GENERAL CONDITIONS

  • These Official Rules constitute the entire agreement between participants and Sponsor with respect to this Contest and supersede all prior or contemporaneous understandings, agreements, representations, or warranties, whether written or oral.
  • If any provision of these Official Rules is held to be invalid or unenforceable, that provision will be struck, and the remaining provisions will remain in full force and effect.
  • Sponsor's failure to enforce any provision of these Official Rules shall not constitute a waiver of that provision.
  • This Contest is intended for residents of the State of Florida only and is not directed to persons outside the United States.
  • Sponsor reserves the right to make changes to these Official Rules at any time. Material changes will be posted at Family & Impant Dentistry of Stuart and will apply prospectively.
